Facility Self-Inspection Survey

The Facility Self-Inspection Survey is OPEN.

Introduction

The online self-inspection survey is an annual report conducted for the Division of Risk Management by the entities that are insured by the fund. The survey is a tool for recognizing and eliminating hazards that can significantly affect the public, our co-workers, the property we are charged to preserve, and the Risk Fund. The timely submission of the online survey will qualify entities to a premium credit.


The 2010 Self-Inspection Survey will be available until June 1st, 2010, after that time it will be closed.

Are you a coordinator or surveyor?

Coordinator = A Person responsible for the entire agencies risk management responsibilities including conducting quarterly risk committee meetings and assigning out all the building on the Self-Inspection Survey. New coordinators will be directed to an online registration process, which must be completed before you will be given electronic access to the application.

Surveyor = A person assigned a facility to inspect. This person is in charge of the survey for the facility but not responsible for the entire agency. Surveyors can login after they are notified by their coordinator or by email that they have a building to inspect.


Coordinator

The Online Coordinator Training offers an overview of the Coordinator's role along with explaining how to create a Self-Inspection Survey for specific facilities.

 

The Coordinator Login will bring up a log-in/registration process page. Once completed, you will be brought to the application to begin building surveys and assigning surveyors for your facilities.

In Person Coordinator Training, (The in person training is only for coordinators.)

Coordinator training for the Self-Inspection Survey will be provided throughout the year and available on our training page. Prior to attending a training session, a Coordinator is encouraged to obtain a UMD Authentication of his/her login email address/password if you are employed at a school district, higher education institution, or charter school (state agency employees already have UMD Authentication). In the public citizens and non-state employees section, click over "Create new user account". It is suggested that you click over the "HELP" link prior to starting the process for a brief tutorial. Online training for both coordinators and surveyors is offered below.

Surveyor

The Surveyor Training (Coming Soon) gives an overview of the Surveyor's role and describes how to complete an assigned Self-Inspection Survey for specific facilities.

 

The Surveyor Login will connect you to a page with an introduction letter. After clicking over the log-in button and completing log-in requirements, the surveyor will be able to see his/her facilities and inspectable areas assigned by the Coordinator.

If you have any questions about the survey facility, or what you need to do to complete the survey, please contact your coordinator for your agency. The agency coordinator will be able to answer your specific question about the facility and requirements, the Division of Risk Management does not know how your agency is conducting the survey and might not have the answer for you.
