SELF INSPECTION SURVEY FAQ

General Questions

We developed the survey with you in mind. It will help you identify areas where safety is lacking and assist you in developing an action plan. As you follow through with your inspections and plans, your liability exposure will reduce and employees and guests will be safer. In addition, completing the Self Inspection Survey allows you to avoid a premium penalty.

In the survey software, the “Where Am I” link is designed to help you identify where you are at in the current process. Use it on the actual inspection page and on the Coordinator page.

Surveyor Questions

  • Contact your agency Risk Coordinator. Coordinator information is found under “Coordinators” on the left side of the inspection page.

Click “Submit and Review” at the bottom of the questions to review your survey. When all questions are answered, “Submit to Risk Management” will appear at the bottom of the review page. Once clicked, the screen will change with a message stating the survey has been submitted. Coordinators are able to unlock surveys if changes are necessary.

Once you are logged in as surveyor, select the facility, then print from the “Print questions in PDF format” link at the top of the page.

  • Contact your Loss Control Consultant, or any of the consultants at the Division of Risk Management

  • Use the “email Risk Management” link on the left of the Coordinator page within the online tool.

The inspection software comes with reports which show survey completion percentage and corrective actions. Click on “Reports” on the left hand side of the Coordinator page, select year, and then choose “Summary Report.” A PDF pop up will show how many facilities are complete. You can also go to Step 5, “Review Status.” The year’s surveys are completed when all facilities show locked. If a facility does not show locked, click on the circle next to the building, and click “Review.” The page will show what questions have been answered. If the survey needs locking, simply click “Lock” at the bottom of the page.

Access the Self Inspection Survey by clicking on its link on the Risk Home page under “Take Action.” Click on the “Coordinator Login” link and the login prompt will appear in a new page.

Facilities an organization must inspect are based upon the value of a facility, its contents, and occupancy and life safety concerns. If you feel a facility should be added to the list, please email us at sis@utah.gov with the information.

Surveyor data and facility information is saved for future inspections. Use this tip: enter all surveyor information first, and then make inspection assignments.

Log in as surveyor, select the facility, and then print from the “Print questions in PDF format” link at the top of the page.

**If you would like all the questions available, then login as a Coordinator and click the “Print Self-inspection Survey” link located on the left. This blank survey is 45+ pages.

To un-assign, in the Coordinator page, choose the facility, select the surveyor, and then click the areas with red check marks. You have successfully un-assigned the surveyor!

Make sure the new surveyor is listed as such in Step 1, “Manage Surveyors.” Then, un-assign the current surveyor from all facilities in question. Lastly, select the new surveyor and assign him or her accordingly.
